one-off design submission form

NOTE:

This is NOT a “first come, first served” situation, so please take your time!

Forms received by midnight on same day of the release will all be considered first in line for a design. If I get multiple requests for the same tattoo on the first day, I will select based on how questions are answered on the form.

PLEASE READ ABOUT SCHEDULING AND DEPOSITS BEFORE SUBMITTING A FORM.

  • I am currently scheduling appointments on Mondays, Wednesdays, and Fridays, starting as early as 10am and as late as 2pm. I do not schedule appointments on weekends.

Currently booking 1-3 months out for one-off tattoo designs.

First appointment must be scheduled within 3 months of receipt of your deposit, and the tattoo must be completed within 6 months.

Sessions are usually 2.5-4 hours of actual tattoo time, plus an hour or so of setup and breakdown. Multiple sessions are scheduled 3-4 weeks apart to allow skin to heal.

If you are traveling for your tattoo, 2 sessions may be scheduled in two consecutive days.

You are allowed one free reschedule due to illness. Multiple reschedules may be subject to an additional fee.

  • Deposits are $250 and are due at at the end of a brief consultation. Deposits are non-refundable and non-transferrable but go towards the cost of your tattoo, applied to the final session.

YOU CAN LOSE YOUR DEPOSIT IF:

    • You want to make changes to the design that were not discussed at the consultation.

    • You decide you want a different design.

    • You no-call no-show to your appointment. This may also result in loss of ability to book future appointments.

    • You failed to disclose important relevant information during the consultation that results in the tattoo not working out for you.

    • You are unable to book appointments during my regular scheduling hours (no weekends) or within the 3 month timeframe.

    • You are unable to start or finish the tattoo due to pregnancy, moving, financial hardship, or other unforeseen circumstances.

Check the FAQ page or use my general contact form if you have questions, and allow up to one week for a reply.
